What is Inreach ?

The Inreach Program is a collaborative partnership between Homeless Healthcare (lead agency) and Royal Perth Hospital (RPH). Operating under RPH’s Homelessness Team, the program provides targeted support to people experiencing homelessness who present at the hospital.

The Inreach team includes a nurse from Homeless Healthcare, who delivers medical and nursing care, and specialist Support Coordinators from Ruah, who offer brief intervention and case management. The program aims to provide support, guidance, referrals, and advocacy to help individuals transition out of homelessness.

Who Can Access the Program?

The Inreach Program is available to individuals who meet the following criteria:

• Are inpatients or scheduled outpatients at Royal Perth Hospital
• Have no fixed address
• Are referred directly by Royal Perth Hospital only
